    • 摘要: 目的 观察和分析血清25羟基维生素D3〔25(OH)D3〕水平与冠状动脉病变严重程度的相关性。方法 选择性冠状动脉造影(CAG)诊断的冠心病患者234例;按25(OH)D3水平分为2组:25(OH)D3非缺乏组〔25(OH)D3≥20 ng/ml,n=33〕和25(OH)D3缺乏组〔25(OH)D3<20 ng/ml,n=201〕。冠状动脉狭窄程度采取国际通用的Gensini评分系统积分;全自动生化分析仪测定空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C),采用串联质谱法检测25(OH)D3。结果 25(OH)D3缺乏组的冠脉病变支数及Gensini积分均显著高于非缺乏组(P<0.05)。Logistic回归分析显示年龄、FPG及25(OH)D3水平是冠脉病变的独立相关因素。Gensini积分与年龄、FPG、LDL-C及TC呈正相关,与25(OH)D3水平呈负相关。结论 25(OH)D3水平是冠心病冠脉病变严重程度的相关因素,二者呈负相关。

       

      Abstract: AIM To observe and to analyze the effect of serum 25-hydroxyvitamin D3 level (25OHD3) on severity of coronary artery lesions. METHODS Two hundred and thirty-four patients with coronary heart disease diagnosed by selective coronary angiogram (CAG) were divided into two groups by their level of 25(OH)D3: non-deficiency group [25(OH)D3≥20 ng/ml, n=33] and 25(OH)D3 deficiency group [25(OH)D3<20 ng/ml, n=201]. The degree of coronary artery stenosis was integrated by an internationally universal used Gensini Scoring System. The study used a fully automatic biochemistry analyzer to measure fasting blood glucose level (FPG), total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL-C) and low density lipoprotein (LDL-C). The investigation also used tandem mass spectrometry to measure 25(OH)D3 level. RESULTS The number of coronary lesions and Gensini score in the 25(OH)D3 deficiency group were significantly higher than those in the non-deficiency group (P<0.05). Logistic regression analysis showed that age, FPG and 25(OH)D3 level were independent relative factors of coronary artery lesions. Gensini integration was positively correlated with age, FPG, LDL-C and TC and it was negatively correlated with 25(OH)D3 level. CONCLUSION 25(OH)D3 level is a relative factor of the severity level of coronary artery lesions and they are negatively correlated.
