    Correlation study of aldosterone-to-renin ratio with nocturnal blood pressure in patients with essential hypertension

    • 摘要: 目的 探讨日常服用降压药的原发性高血压患者醛固酮/肾素比值(ARR)与夜间血压的相关性。方法 入选研究对象179(男88,女91)例,按ARR采用中位数(M)分为2组,比较2组的日间收缩压(DSBP)、日间舒张压(DDBP)、夜间收缩压(NSBP)、夜间舒张压(NDBP)以及在服药物、血钾、血钠等指标。结果 2组中NSBP、NDBP、血钾、血钠有显著性差异(均P<0.05,P<0.01),而性别、年龄、糖尿病、吸烟、体质量指数、DSBP、DDBP以及服用降压药物的差异均无统计学意义。同时校正了相关因素后,ARR与NSBP、NDBP、血钠呈正相关(r=0.528、r=0.481、r=0.342,均P<0.05),而与血钾呈负相关(r=-0.354,P<0.05)。多因素Logistic回归分析显示NSBP、NDBP均是ARR的独立相关因素(P<0.05),0R值分别为1.094(95%CI:0.906-1.205)、1.150(95%CI:0.919-1.445)。结论 原发性高血压患者中ARR与夜间血压显著相关。

       

      Abstract: AIM To study the correlation between aldosterone/renin ratio (ARR) and nocturnal blood pressure in patients with essential hypertension. METHODS According to diagnostic and screening criteria, the study included 179 people (88 male and 91 female). The ARR of all subjects, according to the median, were divided into two groups. Monitoring was performed of 24 hour ambulatory blood pressure, serum potassium, serum sodium and other electrolytes. Daytime systolic blood pressure (DSBP), daytime diastolic blood pressure (DDBP), nocturnal systolic pressure (NSBP), nocturnal diastolic pressure (NDBP), medications, serum potassium, serum sodium and other electrolytes were compared between the two groups. RESULTS There were significant differences between the two groups in NSBP, NDBP, blood potassium, sodium (P<0.05, P<0.01), while gender, age, diabetes mellitus, smoking, BMI, DSBP, DDBP, and medicines were not significantly different. After correcting related factors, ARR was positively correlated with DSBP, DDBP, serum sodium (r=0.528, r=0.481, r=0.342, all P<0.05), and negatively correlated with serum potassium (r=-0.354, P<0.05). Multivariate Logistic regression analysis revealed that NSBP and NDBP were independent risk factors for ARR P<0.05), and the 0R values were 1.094 (95%CI:0.906-1.205) and (95%CI:0.919-1.445), respectively. CONCLUSION In patients with essential hypertension, ARR is significantly correlated with nocturnal blood pressure.
