    • 摘要: 目的:测定血浆和肽素(copeptin)在急性心肌梗死(AMI)组患者中的浓度变化以及与冠脉病变的关系。方法: 对2010年1月~12月我院因胸部不适收治的疑是AMI的患者110例,采用ELISA分析法检测血浆copeptin水平,用固定层析免疫法测定肌钙蛋白、肌酸激酶同工酶(CK-MB)水平。结果: AMI组患者血浆copeptin水平显著高于不稳定心绞痛组和非冠心病组(P<0.01);AMI患者copeptin浓度随着出现症状时间的增加而下降,而肌钙蛋白、CK-MB浓度随着临床症状出现时间的增加而升高;3支病变Copeptin水平明显高于与1、2支病变组。结论: 早期测定血浆copeptin水平对于怀疑为AMI患者的诊断具有重要临床意义,且升高程度可反映冠状动脉病变严重程度。

       

      Abstract: AIM:To determine the plasma copeptin concentration in patients with acute myocardial infarction (AMI) and coronary artery disease (CAD) and to explore its clinical value. METHODS: One hundred and ten patients admitted from January 2010 to December 2010 to our hospital due to chest discomfort were included in the study. Plasma copeptin level was detected using enzyme immunoassay assay and levels of troponin and CK-MB were determined using fixed chromatography immunoassay. RESULTS: Plasma copeptin levels in AMI patients were significantly higher than those in patients with other diseases (P<0.01). In AMI patients, copeptin concentrations decreased with the increasing time of symptoms, whereas TNT CK-MB concentrations increased. Copeptin levels in patients with three-vessel lesions were significantly higher than those in patients with one- or two-vessel lesions. CONCLUSION: Early determination of plasma copeptin level is of important clinical and diagnostic value for suspected AMI patients with subclinical symptoms. Elevated plasma copeptin level may reflect CAD.
