    Correlation of myocardial enzymes and cardiac function in rabbits with hyperthyroid cardiomyopathy

    • 摘要: 目的:探讨兔甲亢性心肌病不同左心室构型血清心肌酶的变化及其与心功能的相关性。方法:20只兔腹腔注射左旋甲状腺素建立甲亢性心肌病动物模型后,依据Ganau的超声参数分为两个组:即向心性肥厚组(CH组)和离心性肥厚组(EH组),另设一组(对照组)兔(10只),每日腹腔注射生理盐水共4周。对各组兔均行超声心动图检查,M型超声测量胸骨旁左室长轴切面左室舒张末期内径(LVEDD)、室间隔厚度(IVSTd)、左心室后壁厚度(PWTd)、左心室射血分数(LVEF)及左心室短轴缩短率(LVFS),公式计算左心室质量(LVM)、左室质量指数(LVMI)、相对室壁厚度(RWT)。测定血清心肌酶的变化:天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)及同功酶、α羟丁酸脱氢酶(α-HBDH)和肌酸激酶(CK)及同工酶(CK-MB)。结果:CH组LVMI及RWT明显增加(P<0.01)。EH组的LVMI明显增加(P<0.01),LVEF、LVFS与对照组比较明显减低(P<0.01)。EH组与对照组及CH组比较,血清CK-MB的水平明显增高(P<0.01),其余心肌酶指标各组两两比较差异均无统计学意义。病理学检查结果显示,EH组损伤较CH组重。血清CK-MB的水平与LVEF和FS呈显著的负相关(r分别为-0.393和-0.423,P<0.05)。结论:不同左室构型的甲亢性心肌病兔血清CK-MB的水平具有不同程度的升高,并与病理损害的程度有关,与心功能不全具有显著相关性。

       

      Abstract: AIM:To study the correlation between the level of serum myocardial enzyme and cardiac function in rabbits with hyperthyroid cardiomyopathy. METHODS: Thirty New Zealand purebred rabbits were randomly divided into two groups: control group (n=10) and experimental group (n=20). Animal model of hyperthyroidism was established by peritoneal injection of levothyroxine (LThy) in the experimental group. According to the ultrasound parameter changes, ventricular geometries of the experimental group were divided into two groups: concentric hypertrophy (CH) and eccentric hypertrophy (EH). Each group underwent conventional echocardiography and myocardial enzyme values were evaluated. RESULTS: In the CH group, LVMI and RWT increased in an obvious manner (P<001). In the EH group, LVMI increased in an obvious manner (P<001), whereas RWT did not change significantly. Both EF and FS in the EH group were significantly lower than those in the control group (P<001). Serum CKMB in the EH group was conspicuously higher than in the control group and CH group (P<001). Pathological examination showed that the damage in the EH group was more severe than in the CH group. LVEF and LVFS were negatively correlated with myocardial enzyme in blood (r=0393, P<005; r=0423, P<005). CONCLUSION: Serum CKMB in different geometric patterns of the left ventricle in rabbits with hyperthyroid cardiomyopathy increases to different degrees. Elevation of myocardial enzyme is associated with the degree of pathological injury and is significantly correlated with cardiac dysfunction.
