    • 摘要: 目的: 观察小型猪自体骨髓内皮祖细胞(Endothelial progenitor cells,EPCs)体外培养扩增后移植对改善心肌梗死(MI)后心室重构的影响并探讨其可能的机制。方法: 将22头小型猪采用经皮冠状动脉球囊堵闭法建立心前壁MI模型。将存活的17头动物喂养2周后,随机分为EPCs移植组(n=10)和对照组(n=7)。移植组EPCs分离并体外培养扩增1周后注入梗死相关血管,对照组分离EPCs后注入培养基至梗死相关血管。继续喂养4周后,采用超声心动图、左心室造影评价心脏结构和功能。用流式细胞仪测定梗死边缘区心肌细胞的凋亡率。结果: EPCs移植组的左心室舒张末内径(LVEDD)较移植前明显缩小(P<0.01),左心室射血分数(LVEF)较移植前明显增加(P<0.01);对照组的LVEDD较移植前有所增加(但P>0.05),LVEF无明显变化。EPCs移植组梗死区室壁的厚度率和梗死边缘区细胞的凋亡率均较对照组明显降低(P<0.05)。结论: EPCs移植治疗可减弱梗死后心室扩张和室壁变薄程度,减少心肌细胞凋亡,进而改善MI后心室重构过程。

       

      Abstract: AIM:To investigate the effects of bone-marrow-derived endothelial progenitor cell (EPC) transplantation on left ventricular (LV) remodeling after myocardial infarction in swine and its potential mechanism. METHODS: The left anterior descending coronary arteries of swine were obstructed by balloon to create myocardial infarction models. Two weeks later, EPCs were collected from bone marrow of the swine by density gradient centrifugation, cultured and amplified in vitro for 1 week. EPCs or medium was then injected into the infarction-related coronary arteries through balloon catheter. After 4 weeks, cardiac function was measured by echocardiography and ventriculography, and the apoptosis rates were assessed by flow cytometry. RESULTS: At 4 weeks after transplantation, LV end-diastolic dimension decreased in the transplantation group (P<0.01) but increased in the control group. Left ventricular function was improved greatly in the transplantation group (P<0.01) but no obvious changes were observed in the control group. The apoptosis rates in the border infarct regions decreased in the transplantation group compared with those in the control group (P<0.05). CONCLUSION: EPC transplantation attenuates the degree of post-MI LV dilation and the development of infarction area and reduces the apoptosis rates of myocardium in the border infarction regions, which may be responsible for the improvement of ventricular remodeling following myocardial infarction.
