    Evaluation of right ventricular longitudinal function in patients with alcoholic cardiomyopathy by twodimensional strain imaging

    • 摘要: 目的: 应用二维应变成像(2DSI)技术评价酒精性心肌病(ACM)右心室纵向功能的临床应用价值。方法: 酒精性心肌病患者32例,正常对照组35例。对两组行常规超声参数及2DSI参数测定,收缩期参数包括:右心室游离壁基底段、中间段和心尖段收缩期的位移峰值(D)、收缩期应变峰值(S)、收缩期应变率峰值(SRs);舒张期参数包括:舒张早期应变率峰值(SRe)、舒张晚期应变率峰值(SRa)。结果: 常规超声参数比较,ACM组右心室前后径(RVEDd)与对照组比较明显增加(P<0.05),两组间右室射血分数(RVEF)无统计学差异。2DSI参数比较,ACM组D、S、SRs、SRe和SRa等参数与对照组比较均显著减低(P<0.05,P<0.01)。结论: 2DSI可作为有效评价酒精性心肌病右心室纵向功能的一种无创性的新方法。

       

      Abstract: AIM:To evaluate the right ventricular longitudinal function with twodimensional strain imaging (2DSI) in patients with alcoholic cardiomyopathy (ACM). METHODS: Conventional echocardiographic parameters and 2DSI parameters were examined in 32 ACM patients (ACM group) and 35 healthy adults (control group). 2DSI parameters were recorded from the displacement (D), strain (S) and strain rate (SRs) curves, which were obtained from basal to apical right ventricular free wall in apical fourchamber using 2DSI. Parameters of peak systolic longitudinal displacement (D), strain (S), systolic strain rate (SRs), early diastolic strain rate (SRe) and late diastolic strain rate (SRa) were included. RESULTS: Compared with control group, RVEDd significantly increased in ACM group (P<005), but no obvious difference was seen in RVEF between groups. Parameters of D, S, SRs, SRe and SRa of all right ventricular free wall segments obtained by 2DSI significantly decreased in ACM group (P<0.05, P<0.01). CONCLUSION: 2DSI provides an effective and noninvasive tool to assess right ventricular longitudinal function in patients with ACM, which has important clinical value.
