    Comparison of three different acoustic contrast agents for detecting right to left shunt in patients with patent foramen ovale by contrast transthoracic echocardiography

    • 摘要: 目的 比较3种声学造影剂对经胸超声心动图声学造影(cTTE)检查卵圆孔未闭(PFO)右向左分流(RLS)的检出效果。方法 随机选取偏头痛及不明原因脑梗死患者84例。所有患者先后用激活生理盐水(AS)、加血激活生理盐水(ASB)和碳酸氢钠维生素B6溶液(VSBS)行cTTE检查,观察有无PFO-RLS及分级。结果 3种造影剂cTTE检查PFO-RLS阳性检出率(阳性率)分别为:AS组25%,ASB组27%和VSBS组36%。VSBS组显著高于AS组和ASB组(均P<0.05)。AS组与ASB组无显著差异。对比AS组PFO-RLS阳性患者3种造影剂检出PFO-RLS的半定量分级显示,ASB组中量和大量RLS阳性率均高于AS组(P<0.05),而VSBS组大量RLS阳性率显著高于AS组和ASB组(均P<0.05)。结论 用VSBS行cTTE检查,对PFO-RLS检测的敏感性及显影效果均优于AS和ASB。

       

      Abstract: AIM To explore the detection sensitivity and developing effect of three kinds of acoustic contrast agents in right heart contrast echocardiography for diagnosis of patent foramen ovale (PFO). METHODS Eighty-four patients with migraine and cryptogenic stroke (aged 18 to 65 years) were enrolled and the right heart contrast echocardiography was conducted using three contrast agents. Real-time observation and comparison of the appearance of microbubble and microbubble density determined whether PFO with right to left shunt (RLS), and the detection rate and the developing effect of PFO using three contrast agents were compared. RESULTS All 84 patients underwent right heart contrast echocardiography examinations three times using three contrast agents. The positive rate of detection was 25% in the agitated saline solution (AS) group, 27% in the agitated saline solution with blood (ASB) group, and 36% in the Vitamin B6 and sodium bicarbonate solution (VSBS) group, which was higher than those of the former two groups. For developing effects, the VSBS group ranked top, followed by the ASB group, and the two groups were better than the AS group. CONCLUSION For detection sensitivity and developing effect, Vitamin B6 and sodium bicarbonate solution is superior to agitated saline solution and agitated saline solution with blood in right heart contrast echocardiography for diagnosis of patent foramen ovale, which is worthy of clinical popularization and application.
