    心力衰竭患者血浆AVP、尿液AQP2与LVEF及低血钠症的关系

    Relationship between plasma arginine-vasopressin and urinary aquaporin-2 and left ventricular ejection fraction and hyponatremia in patients with congestive heart failure

    • 摘要: 目的:探讨充血性心力衰竭(CHF)时,血浆精氨酸加压素(AVP)、尿液水通道蛋白-2(aquaporin-2,AQP2)与左室射血分数(LVEF)及低钠血症的关系。方法: 分别用双抗体夹心酶联免疫吸附测定(ELISA)法和放射免疫分析法同步检测265例充血性心力衰竭患者和92例健康对照者的尿液AQP2及血浆AVP水平,并测定血钠、LVEF等指标。结果: CHF及低钠血症患者血浆AVP、尿液AQP2的浓度比对照组明显增高(均P<0.01),且LVEF越小、血清钠越低,血浆AVP、尿AQP2的浓度越高。结论: AQP2较AVP更能反映机体水代谢紊乱程度,尿液AQP2有望成为判断心力衰竭及水钠失衡严重程度的床旁检测指标。

       

      Abstract: AIM:To study how plasma arginine-vasopressin (AVP) and urinary aquaporin-2 (AQP2) are related with left ventricular ejection fraction (LVEF) and hyponatremia in patients with congestive heart failure (CHF). METHODS: Double antibody sandwich enzyme-linked immunosorbent assay (ELISA) and radioimmunoassay synchronous detection were conducted to examine and compare urine AQP2 and plasma AVP levels as well as serum sodium and LVEF in 265 CHF patients and 92 healthy subjects. RESULTS: The levels of plasma AVP and urinary AQP2 in CHF patients were higher than those in healthy controls and serum sodium concentration and LVEF were negatively correlated. CONCLUSION: Compared with AVP, AQP2 can better reflect water metabolism disorder and urinary AQP2 may be used as a bedside index for detection of severity of heart failure and imbalance of water and sodium.
