Relationship between toll like receptors 4 and the severity of coronary artery lesion in patients with coronary heart disease

摘要: 目的探讨冠心病患者Toll样受体（toll like receptors，TLR）4水平与冠状动脉（冠脉）病变严重程度的相关性。方法选取我科住院经冠状动脉造影确诊为冠心病的患者141例。采用Syntax评分反映冠脉病变严重程度，根据Syntax评分结果分为低分(0~22分)61例、中分(23~32分)41例和高分(≥33分)39例。据此分为2组：Syntax低中分组(n=102)，Syntax高分组(n=39)。通过线性相关分析确定TLR4水平与冠脉病变严重程度的关系，通过多因素Logistic回归分析确定冠脉病变的相关因素。结果与低中分组比较，高分组TLR4水平明显升高〔(93±24) vs.(128±29) ng/ml，P<0.01〕。TLR4水平与Syntax评分呈正相关(r=0.539,P<0.01)。多因素回归分析显示，TLR4水平是Syntax评分≥33分的相关因素(OR=15.645，95%CI：5.322-45.994；P<0.01）。结论 TLR4与冠脉病变严重程度密切相关，随着TLR4水平升高，冠脉病变越严重。

Abstract: AIM To explore the relationship between toll like receptors 4 (TLR4) level and severity of coronary artery lesions in coronary heart disease patients. METHODS 141 patients with coronary heart disease participated in the study. The coronary heart disease group was divided into <33 Syntax score group (low-middle score group, n=102), and ≥33 Syntax score group (high score group, n=39). The correlation between TLR4 levels with Syntax score and risk factors were measured by linear correlation analysis and multivariate regression analysis, respectively. RESULTS Compared with the low-middle score group, TLR4 levels were significantly higher ［(93±24) vs.(128±29) ng/ml, P<0.01］ in high score group. TLR4 levels were positively related to Syntax score (r=0.539, P<0.01). Multivariate regression analysis revealed that TLR4 was independent to the Syntax score≥33 (OR=15.645, 95%CI:5.322-45.994; P<0.01). CONCLUSION TLR4 level is closely associated with severity of coronary artery 1esions.
