    • 摘要: P波离散度(PWD)增大是预测心房颤动(AF)的一种指标。PWD增大的具体机制目前尚不清楚,但大量的临床研究发现,PWD增大的患者有着共同的病理生理基础,主要包括:血流动力学异常、心肌代谢异常、全身炎症反应、氧化应激及自主神经张力增高,而这些病理生理基础对机体心血管功能的损害具有缓慢性、隐匿性、渐进性及不可逆性。研究表明,PWD值的变化与心血管功能的损害具有相关性。本文旨在探讨PWD的变化在心血管疾病的诊断及治疗过程中的意义及价值。

       

      Abstract: Increase of P wave dispersion (PWD) is an index of atrial fibrillation but the mechanism of PWD increase is not clear. A large number of clinical studies have shown that patients with increased PWD shared some pathophysiological pathways including abnormality of flow dynamics, myocardial metabolic abnormality, inflammatory reaction, oxidative stress, and increase of autonomic nervous tension. Damage of cardiovascular functions thus caused is irreversible, occult, progressive and slow. As the change of PWD is associated with cardiovascular dysfunction, this paper aims at discussing the value of diagnosis and treatment of PWD changes in cardiovascular diseases.
