    原发性高血压并发初发糖尿病的相关因素

    An analysis of correlative factors of hypertension with newly diagnosed diabetic patients

    • 摘要: 目的 分析原发性高血压并发初发糖尿病的相关因素。 方法 搜集2013年6月~2016年6月陕西省人民医院心内科收住入院诊断高血压病并发从未使用药物治疗的新诊断2型糖尿病患者468例作为并发糖尿病组。与同期入院诊断单纯原发性高血压患者468例作为对照组(非糖尿病组)比较分析。 结果 并发糖尿病组与非糖尿病组比较,体质量指数〔(28.8±3.9) kg/m2 vs.(25.1±3.2) kg/m2P<0.05〕、HbA1c〔(9.4±1.6)% vs.(6.0±0.4)%,P<0.05〕、总胆固醇(TC)〔(4.8±1.4) mmol/L vs.(3.2±0.8) mmol/L,P<0.05〕、三酰甘油(TG)〔(2.2±1.3) mmol/L vs.(1.6±0.8) mmol/L,P<0.05〕均具有统计学意义。多因素Logistic分析回归分析显示TC、TG偏高为原发性高血压并发初发糖尿病危险因素,OR值分别为2.349(95%Cl:2.10-3.308)、1.903(95%Cl:1.508-2.267);以HbA1c为因变量,体质量指数、TC、TG、低密度脂蛋白胆固醇为自变量行重线性回归分析,HbA1c与TC成正相关,回归方程为:HbA1c=1.309+1.626×TC,P<0.01。 结论 TG、TC水平升高是高血压病并发初发糖尿病患者的危险因素,并且HbA1c与TC呈正相关。

       

      Abstract: AIM To investigate the correlative factors of hypertension complicated with newly diagnosed diabetes. METHODS A total of 468 patients with hypertension complicated with newly diagnosed type 2 diabetes were selected for study group and 468 patients with simple essential hypertension served as controls. RESULTS Significant differences were found in body mass index (28.8±3.9) kg/m2 vs.(25.1±3.2) kg/m2, P<0.05, HbA1c (9.4±1.6)% vs.(6.0±0.4)%, P<0.05, total cholesterol (4.8±1.4) mmol/L vs.(3.2±0.8) mmol/L, P<0.05 and triglyceride (2.2±1.3) mmol/L vs.(1.6±0.8) mmol/L, P<0.05 between study group and controls. Logistic regression analysis showed that total cholesterol and triglyceride were high risk factors for primary hypertension complicated with newly diagnosed diabetes mellitus OR=2.349 (95%Cl:2.10-3.308), OR=1.903 (95%Cl:1.508-2.267). Multiple linear regression indicated that HbA1c was associated with total cholesterol. CONCLUSION Triglyceride and total cholesterol are the risk factors of hypertension complicated with newly diagnosed diabetes mellitus and HbA1c is positively correlated with total cholesterol.
