    应用TIMI计帧法评估扩张型心肌病患者冠脉血流储备及与超敏C反应蛋白的关系

    Assessment of coronary blood flow in dilated cardiomyopathy with TIMI frame count and relationship with high-sensitivity C-reactive protein

    • 摘要: 目的:应用校正的TIMI计帧法(CTFC)来评价扩张型心肌病(DCM)患者的冠脉血流储备(CFR),同时测定超敏C反应蛋白(hs-CRP)的浓度,探讨冠脉血流储备与炎症反应的关系。方法:研究入选31名DCM患者,同时选取29名年龄、性别匹配的健康人作为对照组。两组入选者经冠脉造影证实冠脉形态正常,采用CTFC法测定冠脉的血流速度,采血检测hs-CRP。结果:DCM组主要冠状动脉的TIMI帧数明显高于对照组前降支:(36±7)帧vs.(27±7)帧,回旋支:(37±7)帧vs.(30±7)帧,右冠状动脉:(37±7)帧vs.(30±7)帧,均P<0.01。平均TIMI帧数与hs-CRP呈正相关(R=0.721,P<0.01)。结论:CTFC法能够准确有效地评估DCM患者的CFR,DCM患者冠脉TIMI计数显著高于正常对照组,且CFR的受损程度与hs-CRP呈正相关。

       

      Abstract: AIM:To evaluate coronary blood flow(CFR) by TIMI frame count(FC) in patients with dilated cardiomyopathy(DCM) who had angiographically proven normal coronary arteries and to compare the results with those of healthy controls and observe the relationship between CFR and high-sensitivity C-reactive protein(hs-CRP).METHODS:Thirty-one DCM patients and 29 healthy control subjects without DCM were involved in this study.All patients and control subjects had angiographically proven normal coronary arteries.TIMI FC was determined for each major coronary artery in each patient and hs-CRP was examined at the same time.RESULTS:TIMI FCs for each major epicardial coronary artery in DCM patients were found to be significantly higher than those in control subjects(left anterior descending coronary artery:(36±7) vs.(27±7) frame;left circumflex coronary artery:(37±7) vs.(30±7) frame;right coronary artery:(37±7) vs.(30±7) frame,respectively,P<0.01.Mean TIMI FC was positively correlated with hs-CRP(R=0.721,P<0.01).CONCLUSION:TIMI FC can be used as an effective method to evaluate CFR in DCM patients.FCs in DCM patients are significantly higher than those in healthy control subjects,and impaired CFR is positively correlated with the inflammatory marker hs-CRP.
