    孕妇血清细胞因子及同型半胱氨酸水平与妊娠期高血压疾病的相关性研究

    Relationship between serum cytokine and homocysteine levels in pregnant women and hypertension during pregnancy

    • 摘要:
        目的  探究孕妇血清细胞因子及同型半胱氨酸(homocysteine,Hcy)表达水平与妊娠期高血压疾病的相关性。
        方法  回顾性分析保定市妇幼保健院2020年~2021年3月收治的113例妊娠期高血压疾病患者的临床资料,另选取同期来我院进行健康体检的70例正常妊娠孕妇作为正常妊娠组;比较两组Hcy、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素4(Interleukin-4,IL-4)、胱抑素C(Cystatin C,CysC)、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)、D-二聚体(D-Dimer,D-D)水平,采用Spearman相关性分析Hcy、TNF-α、IL-4、CysC、hs-CRP、D-D水平与妊娠期高血压疾病的关系。
        结果  ①妊娠期高血压疾病组TNF-α水平显著高于正常妊娠组,妊娠期高血压疾病组IL-4水平显著低于正常妊娠组,P<0.01;②妊娠期高血压疾病组Hcy、CysC、hs-CRP、D-D水平均显著高于正常妊娠组,P<0.01;③Spearman相关性分析表明,妊娠期高血压疾病严重程度与TNF-α(r=0.534)、Hcy(r=0.95)、CysC(r=0.562)、hs-CRP(r=0.988)水平均呈现显著正相关,与IL-4呈现显著负相关(r=−0.516),P<0.01。
        结论  :TNF-α、Hcy、CysC、hs-CRP、IL-4水平与妊娠期高血压疾病密切相关,可作为疾病进展的重要因子,妊娠期对以上指标进行密切观测有助于为疾病的诊断及治疗提供依据。

       

      Abstract:
        AIM  To explore the relationship between the expression levels of serum cytokines and homocysteine (homocysteine, Hcy) in pregnant women and hypertension during pregnancy.
        METHODS  A retrospective analysis of the clinical data of 113 patients with hypertension during pregnancy admitted to our hospital from 2020 to March 2021 was made, including 62 patients with mild preeclampsia and 51 patients with severe preeclampsia, and 70 normal pregnant women came to our hospital for healthy physical examination during the same period served as the control group. Hcy, immune cytokines (tumor necrosis factor-α (TNF-α), interleukin-4 (IL-4), Cystatin C (CysC), hypersensitive C-reactive protein (hs-CRP) and D-Dimer (DD) levels were compared between groups. The relationship between Hcy, TNF-α, IL-4, CysC, hs-CRP and DD levels and hypertension during pregnancy was analyzed using Person correlation analysis.
        RESULTS  ① TNF in pregnancy induced hypertension group- α The level of IL-4 in pregnancy induced hypertension group was significantly lower than that in normal pregnancy group (P<0.01); ② Hcy, CysC, hs CRP and D-D water in pregnancy induced hypertension group were significantly higher than those in normal pregnancy group (P<0.01); ③ Spearman correlation analysis showed that the severity of gestational hypertension and TNF-α (r=0.534), Hcy (r=0.95), CysC (r=0.562) and hs CRP (r=0.988) were significantly positively correlated with IL-4 (r=−0.516), P<0.01.
        CONCLUSION  The levels of TNF-α, Hcy, CysC, hs-CRP and IL-4 are closely related to hypertension during pregnancy and they may be important factors in the progression of the disease. Close observation of the above indicators during pregnancy is helpful for the diagnosis and treatment of the disease.
