    心肌梗死再灌注治疗后发生心肌内出血的机制研究和防治进展

    Intramyocardial hemorrhage after coronary reperfusion therapy of myocardial infarction

    • 摘要: 心肌内出血(intramyocardial hemorrhage,IMH)是急性心肌梗死再灌注治疗的不良心血管事件之一,病理是由于持续的缺血缺氧导致血管内皮受损,红细胞渗漏到肌层。IMH在接受再灌注治疗的急性心梗病人中发生率为41%,严重影响再灌注治疗的成功率。近年来IMH受到了人们的广泛关注,已有研究表明IMH与微血管阻塞(microvascular obstruction,MVO)有关,但对IMH的早期识别及准确评估还有待进一步研究。现就IMH的危险因素、发生机制、评估方法进行阐述,为临床预防IMH的发生提供新的治疗思路。

       

      Abstract: Intramyocardial hemorrhage (IMH) is one of the adverse cardiovascular events of acute myocardial infarction, which is due to the persistent ischemia and hypoxia leading to vascular endothelium dysfunction and leakage of red blood cells into musculature. The morbidity of IMH is 41% among acute myocardial infarction patients receiving reperfusion, which seriously affects coronary reperfusion therapy. In recent years, IMH has received widespread attention and studies have shown that IMH is related to microvascular obstruction (MVO). However, early identification and accurate evaluation of IMH need to be further studied. In this article, we summarize the risk factors, mechanisms and evaluation methods of IMH, intending to provide new therapeutic options for clinical treatment.
