    基于四维超声心动图的高原低氧习服患者的右心室结构和功能评估

    Evaluation of right ventricular structure and function in high altitude hypoxic acclimatization patients based on four dimensional echocardiography

        目的  使用四维超声心动图评估高原低氧习服人群的右心室结构与功能,探究其临床实用价值。
        方法  选取首次进入杂多地区的援青支医队员60名作为对照组,随机选取青海玉树杂多地区某驻军部队官兵60名作为高原低氧习服组。使用四维超声心动图测定两组受试者舒张期末室间隔厚度(IVST)、舒张期右室前壁厚度 (RVAWd)和舒张期右心室左右径(RVd)作为右心室的结构参数;测定肺动脉内径(MPA)、主肺动脉收缩压 (PASP)、右室正常射血分数 (RVEF)、右心室每搏输出量(RVSV)、右心室面积变化分数(RVFAC)、右心室舒张末期容积(RVEDV)、 右心室收缩末期容积(RVESV)、右心室游离壁长轴应变(RVFLS)和室间隔长轴应变(IVSLS)。
        结果  与对照组相比,低氧习服组的RVAWd、RVd及IVSd变化差异无统计学意义;而RVEF、RVFLS、RVFAC参数明显降低,RVSV明显升高,差异均有统计学意义(P<0.01)。RVEDV和RVESV升高不明显,IVSLS则降低不明显,差异无统计学意义;与对照组相比,低氧习服组研究对象的MPA和PASP均有不同程度增加,差异均有统计学意义(P<0.01)。
        结论  高原反应低氧习服人群由于条件作用可出现一定的心肌损伤,运用四维超声心动图能够有效观察到早期的右心功能改变。

       

      Abstract:
        AIM  To evaluate the right ventricular structure and function of high altitude hypoxic acclimatization population by using four-dimensional echocardiography and to explore its clinical value.
        METHODS  A random selection of 60 officers and soldiers of a garrison in the Zaduo area of Yushu was used as the plateau hypoxic training group and 60 members of the Qinghai medical aid team who entered the Zaduo area for the first time were selected as the control group. Four-dimensional echocardiography was used to measure the end diastolic ventricular septal thickness (IVST), diastolic right ventricular anterior wall thickness (RVAWD) and diastolic right ventricular diameter (RVd) as right ventricular structural parameters, pulmonary artery diameter (MPA), main pulmonary artery systolic pressure (PASP), right ventricular normal ejection fraction (RVEF), RVSV, RVFAC, RVEDV, RVESV, RVFLS and IVSLS.
        RESULTS  Compared with those in the control group, RVAWD and RVd were increased and IVST did not change significantly in the hypoxic acclimatization group. Compared with those in the control group, RVEF, RVFLS and RVFAC were significantly decreased and RVSV was significantly increased in the hypoxic acclimatization group (P<0.01). RVEDV and RVESV were not significantly increased and IVSLS was not significantly decreased. Compared with those in the control group, MPA and PASP were increased to varying degrees in the hypoxic acclimatization group and the differences were statistically significant (P<0.01).
        CONCLUSION  Four-dimensional echocardiography more objectively and truly reflects the early changes of right ventricular functions in high altitude hypoxic acclimatization people, which provides data support for screening and prevention of altitude sickness and is worthy of popularization and application.
