    血清Periostin蛋白、肝细胞生长因子对慢性心衰患者心血管事件的预测价值

    Predictive value of serum periostin protein and hepatocyte growth factor for cardiovascular events in patients with chronic heart failure

    • 摘要:
        目的  探讨血清Periostin蛋白、肝细胞生长因子(hepatocyte growth factor, HGF)对慢性心力衰竭(chronic heart failure, CHF)的预测价值。
        方法  选取2018年1月至2019年3月我院收治的CHF患者(n=108),纳入同期体检健康者为对照组(n=50),采用ELISA法测定两组入院时血清Periostin蛋白和HGF 水平。按随访结果是否出现不良心血管事件分为A组(无心血事件,n=50)和B组(有心血管事件,n=58)。采用logistic回归分析CHF患者不良心血管事件的独立危险因素,并通过绘制受试者工作特征曲线(receiver operating characteristic curve, ROC)判定预测价值。
        结果  B组血清Periostin蛋白和HGF水平显著高于A组(P<0.05);且A和B两组血清Periostin蛋白和HGF水平显著高于对照组(P<0.01)。多因素分析结果显示: 年龄、身体质量指数(BMI)、高血压、糖尿病、Periostin蛋白和HGF为CHF患者不良心血管事件的独立危险因素(P<0.05);以Periostin=103.3 1 ng/L,HGF=901.72 pg/ml为切点的预测: ROC曲线下与坐标轴围成的面积AUC为0.915,灵敏度为92.98 %,特异性为92.16 %。
        结论  血清Periostin蛋白与HGF水平的升高,可作为CHF患者不良心血管事件发生的可靠血清标志物。

       

      Abstract:
        AIM  To explore the predictive value of serum periostin protein and hepatocyte growth factor (HGF) for cardiovascular events in patients with chronic heart failure.
        METHODS  We prospectively selected 108 patients with chronic heart failure admitted to our hospital from January 2018 to March 2019 for this study and another 50 healthy patients under physical examination severed as control group. ELISA was used to determine the serum levels of periostin protein and HGF at admission. According to the follow-up results, the patients were divided into group A (without cardiovascular events, 50 cases) and group B (with cardiovascular events, 58 cases). Logistic regression analysis was conducted to explore independent risk factors for cardiovascular events in patients with chronic heart failure. ROC curve was drawn to determine the predictive value.
        RESULTS  The serum levels of periostin protein and HGF in group B were significantly higher than those in group A (P<0.05). And the serum levels of Periostin protein and HGF in A、B groups were significantly higher than those in the control group (P<0.01); Multivariate results showed that age, BMI, hypertension, diabetes, periostin protein and HGF were independent risks of cardiovascular events in patients with chronic heart failure factors (P<0.05). Setting periodin=103.31 ng/L, HGF=901.72 (pg/ml) as the cut point, the predicted AUC was 0.915, the sensitivity was 92.98% and the specificity was 92.16.
        CONCLUSION  The elevated serum levels of periostin protein and hepatocyte growth factor can be used as reliable serum markers for cardiovascular events in patients with chronic heart failure.
