FFR指导下多支血管病变功能性血运重建的临床研究进展

    Research progress of functional revascularization guided by fractional flow reserve in patients with multivessel disease

    • 摘要: 多支血管病变(MVD)冠脉病变复杂,血运重建治疗策略的选择是介入治疗的难点。既往认为MVD解剖学完全血运重建可改善患者预后,但最近研究显示解剖学完全血运重建(PCI或CABG)并不能显著改善患者的预后,血运重建的生存获益与心肌缺血的风险相关,提示应重视缺血相关的功能性血运重建。血流贮备分数(FFR)是判断冠脉狭窄有无功能性缺血的生理学检查指标,能准确识别MVD中真正引起心肌功能性缺血的罪犯血管。本文就FFR指导下MVD功能性血运重建的临床研究进展做一综述。

       

      Abstract: The lesions of multivessel disease are complicated and choice of revascularization strategy is challenging. It has previously been believed that complete revascularization can improve the prognosis of patients with multivessel disease; however, recent anatomic study of complete revascularization has demonstrated that it does not significantly improve prognosis of patients. The prognosis of patients is related to risk of myocardial ischemia, which suggests that functional revascularization should be emphasized. Fractional flow reserve is a physiological index for evaluating coronary artery disease and it can identify the culprit vessels of myocardial ischemia caused by multivessel diseases. The progress of functional revascularization in patients with multivessel diseases is reviewed in this manuscript.
