经导管去肾神经术在高血压及其他疾病治疗中的研究进展

    Research progress in catheter-based renal denervation in treatment of hypertension and other diseases

    • 摘要: 肾交感神经的过度激活,在高血压的发生发展中起着重要作用。自2009年SYMPLICITY HTN-1研究发表以来,经导管去肾神经术(renal denervation,RDN)治疗高血压有了长足发展,临床证据越来越充分,已经被多个国家和地区推荐作为药物以外,治疗难治性高血压或血压控制不佳患者的一种方法。然而,RDN除用于治疗高血压外,对心力衰竭、心律失常等心源性疾病和阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea,OSA)、慢性肾脏病(chronic kidney disease,CKD)及糖脂代谢等非心源性疾病也存在潜在益处。本文旨对RDN 治疗高血压及其他疾病中的临床证据进行阐述,并探讨其局限性及未来的发展方向。

       

      Abstract: The excessive activation of the renal sympathetic nervous system plays a crucial role in the onset and development of hypertension. Since the publication of the SYMPLICITY HTN-1 study in 2009, renal denervation (RDN) via catheter-based procedures has made significant advancements. With increasingly robust clinical evidence, it has been recommended in many countries and regions as a non-pharmacological method for treating refractory hypertension or patients with inadequate blood pressure control. Besides hypertension, RDN holds potential benefits for cardiogenic diseases such as heart failure and arrhythmias, as well as non-cardiogenic diseases including obstructive sleep apnea (OSA), chronic kidney disease (CKD) and metabolic disorders. This article aims to elucidate the clinical evidence of RDN in the treatment of hypertension and other diseases, discuss its limitations and explore future directions.
