血尿酸水平与臂踝脉搏波传导速度的相关性一项基于Dryad数据库的二次分析

    Correlation between serum uric acid levels and brachial-ankle pulse wave velocity: a secondary analysis based on Dryad database

      目的 明确血尿酸(SUA)水平与臂踝脉搏波传导速度(baPWV)之间的相关性。
      方法 基于Dryad数据库数据进行二次分析,通过多元线性回归探究SUA水平与健康体检人群中baPWV之间的线性关系,采用限制性立方样条进行曲线拟合,根据SUA水平三分位数分成Q1组、Q2组和Q3组,进行性别、吸烟、脂肪肝及体质量指数(BMI)亚组分析。
      结果 与Q1组比较,Q2组与Q3组男性比例、吸烟比例及脂肪肝比例更高(均P<0.05),BMI、SBP、DBP、每周饮酒量、AST、ALT、γ-GGT、FPG、TG水平增加(均P<0.01),LDL-C水平增加(Q2组P<0.05,Q3组P<0.01),HDL-C(P<0.01)与eGFR (Q2组 P<0.05;Q3组 P<0.01) 水平降低,ABI在Q2组降低,在Q3组升高(均P<0.01),baPWV (Q2组 P<0.05;Q3组 P<0.01)升高。与Q2组比较,Q3组男性比例、吸烟比例及脂肪肝比例更高(均P<0.05);BMI、SBP、DBP、每周饮酒量、AST、ALT、γ-GGT、FPG、TG、HDL-C、eGFR水平升高(均P<0.01); ABI(P<0.05)水平升高。在调整各混杂因素后,血尿酸与baPWV之间仍呈显著线性相关,血尿酸水平每升高1 mg/dL,baPWV增加12.70 cm/s(β=12.70, 95% CI: 0.83~24.58, P<0.05)。与血尿酸最低三分位数组相比,最高组中baPWV显著增加(β=39.76, 95% CI: 1.17~78.35, P=0.043)。亚组分析显示,在女性、不吸烟者、无脂肪肝及BMI<24 kg/m2亚组中血尿酸与baPWV之间相关性更为显著(均P<0.05)。
      结论 健康体检人群中血尿酸水平升高与baPWV升高相关,应重视动脉硬化风险的早期筛查与干预。

       

      Abstract:
      AIM To investigate the correlation between serum uric acid levels and brachial-ankle pulse wave velocity (baPWV).
      METHODS A secondary analysis was performed utilizing Dryad database and the linear relationship between serum uric acid and baPWV in populations under health check-up was explored by multivariable linear regression. Restricted cubic spline was used for curve fitting and subgroup analysis was performed using the items including gender, smoking, fatty liver disease and body mass index (BMI).
      RESULTS Compared with the Q1 group, the Q2 and Q3 groups had a higher proportion of males, smoking, and fatty liver (all P<0.05). BMI, SBP, DBP, weekly alcohol consumption, AST, ALT γ- The levels of GGT, FPG, TG increased (all P<0.01) and LDL-C increased (Q2 group P<0.05,Q3 group P<0.01), while the levels of HDL-C (P<0.01) and eGFR (Q2 group P<0.05; Q3 group P<0.01) decreased. ABI decreased in Q2 group and increased in Q3 group (all P<0.01), and baPWV (Q2 group P<0.05; Q3 group P<0.01) increased. Compared with the Q2 group, the Q3 group had a higher proportion of males, smokers, and fatty liver (all P<0.05); BMI, SBP, DBP, weekly alcohol consumption, AST, ALT γ- The levels of GGT, FPG, TG, HDL-C, and eGFR increased (all P<0.01); The level of ABI (P<0.05) increased. After adjusting for various confounding factors, there is still a significant linear correlation between blood uric acid and baPWV. For every 1 mg/dL increase in blood uric acid level, baPWV increases by 12.70 cm/s (β=12.70, 95% CI: 0.83~24.58, P<0.05). Compared with the lowest quartile array of blood uric acid, the highest group showed a significant increase in baPWV (β= 39.76, 95% CI: 1.17~78.35, P=0.043). Subgroup analysis showed that the correlation between blood uric acid and baPWV was more significant (all P<0.05) in the female, non-smokers, non fatty liver, and BMI<24 kg/m2 subgroups.
      CONCLUSION The increase in serum uric acid levels is associated with an increased baPWV in healthy people and more attention should be given for early screening and intervention of arteriosclerosis.
