肺动脉高压去神经支配治疗肺动脉高压的研究进展

    Research progress in treatment of pulmonary arterial hypertension with pulmonary denervation

    • 摘要: 从20世纪初开始,大量的临床研究不断证实,交感神经在肺动脉高压(pulmonary arterial hypertension, PAH)的发病机制中扮演着至关重要的角色。PAH与交感神经系统的过度刺激和压力感受器介导的血管收缩密切相关,这导致了病理性肺动脉和右心室重塑。肺动脉去神经支配(pulmonary artery denervation,PADN)是一种微创的干预措施,可消融局部交感神经纤维和压力感受器,以调节病理性血管收缩。初步的动物和临床研究显示,短期肺血流动力学和PA重构的改善。本文综述了交感神经活性增加在PAH发展中的病理生理机制,回顾动物和人体实验文献,描述PADN的微创技术的具体操作,并探索未来的研究方向。

       

      Abstract: Over-activation of the sympathetic nervous system plays an important role in the progression of pulmonary arterial hypertension (PAH). Pulmonary artery denervation (PADN) is currently an interventional method for non pharmacological treatment of PAH. This review introduces the innervation of the pulmonary artery neurohormone axis, as well as the animal studies and clinical trial results of PADN in PAH.
