参仙升脉口服液激活 Nrf2/HO-1 通路抑制内质网应激改善窦房结综合征小鼠窦房结纤维化

    Shenxian Shengmai Oral Liquid activates Nrf2/HO-1 pathway to inhibit endoplasmic reticulum stress and improve sinoatrial node fibrosis in sick sinus syndrome mice

      目的 旨在揭示参仙升脉口服液降低内质网应激,改善病态窦房结综合征(SSS)模型小鼠窦房结纤维化方面的Nrf2/HO-1通路调节机制,为中医药复方在临床防治SSS的应用提供理论支持。
      方法 以50只SPF级C57B6小鼠为研究对象,采用随机数字表法将小鼠分组为5组:假手术组(Sham组)、窦房结综合征模型组(SSS组)、以及参仙升脉口服液低、中、高剂量组(SXSM-L、SXSM-M、SXSM-H组)。利用皮下缓释泵注血管紧张素Ⅱ(AngⅡ)法建立SSS小鼠模型。收集不同时间点心率指标(1 d、7 d、14 d、21 d、28 d),HE、Masson染色观察小鼠窦房结区域(SAN)病理学变化。ELISA检测氧化应激标志物、免疫荧光检测细胞内ROS水平、Western blot检测内质网应激蛋白、Nrf2/HO-1信号通路蛋白表达变化。
      结果 参仙升脉口服液显著提高SSS小鼠心率,并且显著减轻小鼠窦房结损伤和纤维化(P<0.05)。此外,参仙升脉口服液能有效降低SSS小鼠血清中的MDA水平,并促进SOD和CAT的特异表达(P<0.05)。进一步研究还发现,参仙升脉口服液显著上调了SSS小鼠窦房结区域的SERCA2a、GRP78、HO-1、Nrf2、Trx-1蛋白表达,并下调了Caspase-12的表达(P<0.05),其调节作用与药物浓度正相关(P<0.05)。
      结论 参仙升脉口服液可能通过促进Nrf2/HO-1信号通路的活化,有效减缓窦房结细胞中的内质网应激现象,维护细胞内Ca2+的平衡,从而调控窦房结细胞的自主节律,并阻止窦房结的纤维化过程。

       

      Abstract:
      AIM To unveil the mechanisms by which Shenxian Shengmai Oral Liquid mitigates endoplasmic reticulum stress and ameliorates sinus node fibrosis in sick sinus syndrome (SSS) model in mice via Nrf2/HO-1 signaling pathway so as to provide a theoretical foundation for clinical application of traditional Chinese medicine formulations in the prevention and treatment of SSS.
      METHODS Fifty SPF-grade C57B6 mice were selected as experimental subjects and randomly assigned into five groups using a randomized number method: sham operation control group (Sham group), SSS model group (SSS group), and respectively low, medium, and high dosage groups of Shenxian Shengmai Oral Liquid (SXSM-L group, SXSM-M group, SXSM-H group). An SSS mouse model was established using an AngII method with a subcutaneously implanted osmotic pump. Heart rate metrics were collected at various time points (1, 7, 14, 21, 28 days) and pathological changes in the sinoatrial node (SAN) region were observed with HE and Masson staining. ELISA was employed to detect oxidative stress markers, immunofluorescence was used to measure intracellular ROS levels and Western blot was used to assess the expressions of endoplasmic reticulum stress proteins and Nrf2/HO-1 signaling pathway proteins.
      RESULTS Shenxian Shengmai Oral Liquid significantly increased heart rate in SSS mice and markedly alleviated sinus node damage and fibrosis (P<0.05). Furthermore, it effectively reduced the serum levels of MDA in SSS mice and promoted the specific expressions of SOD and CAT (P<0.05). Additional studies revealed that Shenxian Shengmai Oral Liquid significantly up-regulated the expressions of SERCA2a, GRP78, HO-1, Nrf2, and Trx-1 proteins in the SAN region of SSS mice and down-regulated the expressions of caspase-12 (P<0.05), with a regulatory effect that was positively correlated with the concentration of the drug (P<0.05).
      CONCLUSION Shenxian Shengmai Oral Liquid facilitates the activation of the Nrf2/HO-1 signaling pathway, effectively easing endoplasmic reticulum stress in sinus node cells, preserving the balance of intracellular Ca2+ and thus regulating the intrinsic rhythm of sinus node cells and hindering the fibrotic process within the sinus node.
