人参皂苷治疗心肌缺血/再灌注损伤研究进展

    Research progress in role of ginsenosides in myocardial ischemia-reperfusion injury

    • 摘要: 缺血性心脏病(ischemic heart disease,IHD)的最佳治疗策略是尽早恢复心肌血流灌注,但是再灌注会导致缺血心肌的损伤,即心肌缺血/再灌注(myocardial ischemia-reperfusion, MI/R)损伤。MI/R损伤会扩大心肌梗死面积,影响血运重建,严重影响患者预后。因此,采用药物干预以防治MI/R损伤是IHD临床救治的重大需求和目标。传统中药的有效成分为防治MI/R损伤提供了新的用药策略和途径,其中含有人参的制剂被广泛应用于心血管疾病的治疗。人参皂苷是人参的有效成分,研究表明其对MI/R损伤有一定的防治作用。本文对人参皂苷防治MI/R损伤的作用和相关机制进行综述,以期为人参皂苷的临床应用提供理论依据和转化的可能。

       

      Abstract: The best treatment strategy for ischemic heart disease (IHD) is to restore myocardial perfusion as early as possible, but reperfusion can lead to injury of the ischemic myocardium, i.e. myocardial ischemia-reperfusion (MI/R) injury. MI/R injury can enlarge the myocardial infarct area, affect hematologic reconstitution and seriously affect patient prognosis. Therefore, the use of pharmacological intervention and prevention of MI/R injury is a major need and goal of clinical salvage in IHD. The active ingredients of traditional Chinese medicine provide new medication strategies and pathways for the prevention and treatment of MI/R injury. Preparations containing ginseng are widely used in the treatment of cardiovascular diseases and ginsenosides are the active ingredients of ginseng. The mechanism of action of ginsenosides on cardiovascular diseases has yet to be elucidated and it is therefore necessary to explore the cardioprotective effects of ginsenosides in depth. This paper reviews the mechanisms associated with ginsenosides in the treatment of MI/R injury, providing a theoretical basis and translational possibilities for the clinical application of ginsenosides.
