AIM:To improve the life quality and reduce medical costs of elderly patients with chronic heart failure by community-and-hospital seamless management and traditional drug treatment. METHODS: Patients with chronic heart failure in the community were divided into two groups: experimental group and control group, and the differences between the two groups were compared after two years’ time. RESULTS: Statistical differences were found in NYHA grades, left ventricle ejection fraction, and left ventricular end-diastolic diameter between experiment group of community-and-hospital seamless management and traditional drug treatment and control group (P<0.01). CONCLUSION: Community-and-hospital seamless management improves the therapeutic effect of the treatment and reduces the medical costs in patients with chronic heart failure.
