AIM:To explore the effect of community and hospital seamless management on quality of life and family health education in elderly patients with chronic heart failure (CHF). METHODS: Elderly patients with CHF in the community were divided into two groups according to residential areas: experimental group of community and hospital seamless management and control group of only drug treatment. Differences between groups were compared after 2 years. RESULTS: Medication compliance, quality of life and family health education in experiment group were significantly better than those in the control group (P<0.01). CONCLUSION: Community and hospital seamless management markedly improves medication compliance, quality of life and family health education in elderly patients with CHF.
