    Predictive value of platelet distribution width for coronary artery disease in young patients

    •   AIM   To investigate the distribution of platelet distribution width (PDW) in young patients with coronary artery disease (CAD) and its relationship with the severity of coronary artery disease, and to evaluate the predictive value of PDW for young CAD patients.
        METHODS   Young patients (ages 14 to 44 years) admitted to the Department of Cardiology of our hospital from October 2018 to October 2019 were studied. All patients received coronary angiography (CAG) and they were divided into the CAD group (n=240) and the control group (n=88, without CAD). The PDW levels were compared between the two groups and the subgroups of CAD group. The correlation between PDW and severity of CAD (Gensini score) and independent risk factors for CAD were analyzed.
        RESULTS   PDW level in CAD group was significantly higher than that in the control group (P<0.05). PDW in acute myocardial infarction (AMI), unstable angina (UAP) and stable angina (SAP) groups was higher than the control group (P<0.05). There was a positive correlation between PDW and Gensini scores in CAD group (r=0.371, P<0.01). Multivariate logistic regression analysis showed PDW (OR=2.757, 95% CI: 2.117-3.590, P<0.01) was an independent risk factor for young CAD patients. The boundary value of PDW to predict CAD in young patients was 15.65%. The area under the ROC curve was 0.87 (95% CI: 0.826-0.914, P<0.01), sensitivity 84% and specificity 82%.
        CONCLUSION   PDW is closely related to the severity of coronary artery disease in young CAD patients. It is an independent risk factor for young CAD patients and is of predictive value for diagnosis of CAD in young patients.
