    • 摘要: 目的 观察左卡尼汀(L-carnitine,L-CN)对高血压并发糖尿病大鼠心肌代谢和心功能的保护作用。方法 采用两肾一夹手术及腹腔内注射链脲佐菌素(STZ)的方法,制备高血压并发糖尿病大鼠模型。实验分为3组,即正常对照组、高血压糖尿病模型组及高血压糖尿病治疗组,每组10只。10周后,检查射血分数(EF)、左室短轴缩短率(FS)、左室收缩压(LVSP)及左室舒张末压(LVEDP)。测定血总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血糖(GLU)、血游离脂肪酸(FFA)的浓度,并计算心脏指数(CI=HW/BW), 观察心肌改变。结果 经10周治疗,①高血压糖尿病模型组与对照组相比, TC、TG、LDL-C、GLU、FFA的水平、CI及LVEDP明显升高(P<0.05);HDL-C、EF、FS和LVSP明显下降(P<0.05)。HE染色显示,心肌细胞肥大、排列紊乱、肌纤维断裂、溶解。电镜显示,肌纤维局灶性断裂、溶解、坏死,线粒体空泡化。②高血压糖尿病治疗组与模型组相比, LDL-C、GLU、FFA及LVEDP明显下降(P<0.05);EF、FS及LVSP明显升高(P<0.05)。HE染色显示,肌纤维排列相对规则,未见明显的细胞肥大排列紊乱、肌纤维断裂、溶解。电镜显示,心肌的超微结构大致正常。结论 L-CN可减轻高血压-糖尿病大鼠的心肌糖、脂代谢的紊乱,降低血中FFA的浓度,保护心肌细胞和心脏的功能。

       

      Abstract: AIM To observe the protection of L-carnitine (L-CN) on myocardial metabolism and heart function in hypertensive rats complicated by diabetes. METHODS The method of operation (two kidney one clip) plus streptozotocin(STZ) injection was adopted to set up the model of hypertensive rats complicated by diabetes(HP-DM). Thirty male SD white rats were divided into three groups at random: normal group(n=10), HP-DM model group(n=10) and HP-DM treatment group(n=10). After 10 weeks, left ventricular ejection faction(EF), fractional shortening (FS), left ventricular systolic pressure(LVSP) and left ventricular end-diastolic pressure(LVEDP) were determined by echocardiography. Serum level of glucose(GLU), total cholesterol(TC), triglyeride(TG), low-density lipoprotein cholesterol(LDL-C), high-density lipoprotein cholesterol(HDL-C) and free fatly acids(FFA) were measured. The cardiac index(CI=HW/BW) was computed. Myocardium changes were observed by HE staining and electron microscope respectively. RESULTS After10 weeks’ treatment, higher serum levels of TC, TG, LDL-C, GLU, FFA, CI and LVEDP (P<0.05), and lower HDL-C, EF, FS and LVSP(P<0.05) were observed in the HP-DM model group in comparison with those in the control group. In the HP-DM group, HE staining revealed that hyertrophy and disordered arrange of myocardial cells, fracture and dissolution of muscle fibers and nuclear fission. Focal fracture, dissolution and necrosis of muscle fibers and swelling and vacuolization of mitochondria could be observed under electron microscopy. Compared with those in the HP-DM model group, the serum levels of LDL-C, GLU, FFA and LVEDP decreased significantly(P<0.05) and the EF, FS and LVSP increased significantly(P<0.05) in the HP-DM treatment group. HE staining revealed visible hyertrophy and disordered arrange of myocardial cells. Fracture and dissolution of muscle fibers and nuclear fission, and cardiac muscle ultrastructure was generally normal under electron microscopy in the L-CN treatment group. CONCLUSION L-CN assuages the cardiocyte energy metabolism disorder, decreases the plasma FFA concentration and thus protects the cadiocytes and cardiac function.
