Abstract: Chronic heart failure (CHF), a common chronic cardiovascular disease, is the last stage of various cardiovascular diseases. With increased life expectancy of the population, the incidence of chronic heart failure increases each year and has become a major public health problem. The establishment of an integrated hospital-community-patient-based trinity management can effectively enhance the patients quality of life, reduce health care costs, lower readmission rates and improve the overall treatment effect.
