    • 摘要: 目的 在冠状动脉造影(CAG)的老年患者中观察并发肾动脉狭窄(RAS)的发生率,并分析并发RAS的影响因素。方法 CAG同时行肾动脉造影(RAG)患者277例,分析动脉粥样硬化的危险因素与RAS发生的关系。结果 经CAG证实冠心病患者212例,RAS发生率19%,CAG正常患者RAS发生率8%,冠脉多支病变组中RAS发生率明显高于单支病变组及冠状动脉正常组(P<0.01),多因素分析,独立预测因子为冠状动脉病变程度(OR=1.89,95%可信区间:1.35-2.65,P<0.01)。结论 冠心病患者有较高RAS发生率,发现冠状动脉多支病变患者应常规行RAG检查。

       

      Abstract: AIM: To investigate incidental renal artery stenosis (RAS) in elderly patients under coronary artery angiography and to analyze the risk factors of RAS. METHODS: Renal artery angiography (RA) was performed in 277 patients following coronary angiography to screen for the presence of RAS in order to study the relationship between coronary artery disease (CAD) and RAS. RESULTS: The incidence of RAS was 19% (41/212) in patients with CAD who had been confirmed by coronary artery angiography. Five patients with RAS were found in 65 patients whose coronary arteries were normal (8%, 5/65). RAS incidence was higher in multivessel CAD than single-vessel disease (P<0.01). In multivariate analysis, the severity of CAD (OR=1.89, 95% CI: 1.35-2.65, P<0.01) was independently correlated with RAS. CONCLUSION: RAS incidence is higher in patients with CAD. RA should be performed routinely after patients have been identified with multivessel CAD by coronary artery angiography.
