    • 摘要: 目的: 观察心电图Cornell指数对高血压病左室肥厚以及左室肥厚不同几何构型的诊断价值。方法: 选用本院住院以及门诊高血压病患者96(男50,女46)例,年龄(54±14)岁。根据心脏超声检查的结果,将入选患者分为4组:①正常几何形态(normal geometry,N);②向心性重构(concentric remodeling,CR);③向心性肥厚(concentric hypertrophy,CH);④离心性肥厚(eccentric hypertrophy,EH),观察心电图Cornell指数在各组中的阳性率。结果: Cornell指数均能明显区分正常心脏形态及左心室肥厚;在不同左室肥厚构型中,Cornell电压指数>2.8 mV(男)和Cornell电压指数>2.0 mV(女)以及Cornell乘积指数>2 440 mm/ms能够显著区分CH(阳性率分别为88%、85%和82%)和EH(阳性率分别为62%、60%和62%)(P<0.01);Cornell乘积指数>2 440 mm/ms(在CR组的阳性率(80%、75%)明显高于EH组(62%、40%)(P<0.01)。结论: 心电图Cornell电压指数及Cornell乘积指数可较好鉴别高血压病左室肥厚的不同几何构型。

       

      Abstract: AIM: To assess the diagnostic values of Cornell indexes in diagnosis of left ventricular hypertrophy (LVH) and in detecting different geometric patterns of hypertrophy in patients with essential hypertension. METHODS: Ninety-six patients with essential hypertension were divided into four groups according to the different parameters of echocardiography such as left ventricular mass index and relative wall thickness: normal geometry group (N), concentric remodeling group (CR), concentric hypertrophy group (CH) and eccentric hypertrophy group (EH). Cornell indexes in different groups were analyzed. RESULTS: Cornell voltage index and Cornell product index differentiated LVH from normal geometry. Cornell voltage index >2.8 mv (male), Cornell voltage index >2.0 mv (female) and Cornell product index >2 440 mm/msec (male) were of great value in discriminating CH (88.2%, 84.6% and 82.4%) and EH (62.5%, 60.0% and 62.5%) (P<0.01). Cornell product index >2 440 mm/msec (male and female) was significantly higher in the CR group (80.0%, 75.0%) compared with that in the EH group (62.5%, 40.0%) (P<0.01). CONCLUSION: Cornell voltage index and Cornell product index have significant values in discriminating different geometric patterns of LVH.
